The Wellness & Recovery Specialist is responsible for care coordination activities focused on the Wellness and Recovery of members. Location: Remote. Candidates must reside in the following states: Indiana, Georgia, or Ohio. Peer Specialist Certification required. Responsibilities include identifying opportunities for engagement of members and their families in forming a supportive, recovery network; developing and implementing psychiatric discharge planning education at Recovery and Resiliency sites; collaborating with Stabilization Teams as a member advocate in discharge planning education, resolution of barriers, and service transitions; acting as a resource for staff on decision making and problem solving; initiating and maintaining contact with assigned individuals and providers to determine member's response to services; teaching and role modeling the value of the member’s recovery experience; modeling effective coping techniques and self-help strategies; serving as a recovery agent by providing and advocating for recovery-based services to assist members in daily living; assisting members in working with their case manager or treatment team to achieve goals and self-directed recovery; assisting members in setting up and sustaining self-help groups; assisting members in understanding Peer Recovery resources available within the local community; collaborating with interdisciplinary teams as a member advocate; utilizing tools such as the Wellness Recovery Action Plan (WRAP); assisting clients in developing empowerment skills and combating stigma through self-advocacy. Minimum requirements include AA/AS and minimum of 2 years of experience in health services or behavioral health field or peer support services role, and Peer Specialist Certification. Preferred skills include experience as a Peer Support Specialist, computer proficiency, telephonic customer service, knowledge of recovery process, mental illness and substance use disorders, community resources, health coaching, motivational interviewing, and strong communication skills.
