Job Description

Site: The General Hospital Corporation At Mass General Brigham, we know it takes a surprising range of talented professionals to advance our mission-from doctors, nurses, business people and tech experts, to dedicated researchers and systems analysts. As a not-for-profit organization, Mass General Brigham is committed to supporting patient care, research, teaching, and service to the community. We place great value on being a diverse, equitable and inclusive organization as we aim to reflect the diversity of the patients we serve. At Mass General Brigham, we believe a diverse set of backgrounds and lived experiences makes us stronger by challenging our assumptions with new perspectives that can drive revolutionary discoveries in medical innovations in research and patient care. Job Summary Under the direction of a staff pharmacist and/or pharmacy leadership team, follow established policies and procedures to perform a variety of highly advanced technical and clerical support tasks that are vital to the operation of the pharmacy department. Tasks include responsibilities of technical training, compliance, and participation in departmental projects and initiatives. This requires successful experience and documented performance in focused key activities. Most tasks require the review and authorization of a licensed pharmacist. Medication reconciliation during transitions of care from the community to hospital setting is essential for patient safety and to optimize inpatient care. This process begins with a thorough history of all regular medication use prior to admission (prescribed and non-prescribed), using several different information sources. The medication history pharmacy technician, under the supervision of the medication history pharmacist, will systematically interview the patient, family, and/or caregiver to obtain a complete list of home medications. This medication list will then be confirmed with a second source that may include a pharmacy, mail order, or nursing facility documentation. Following completion, the pharmacy technician will document findings in the medical record and communicate pertinent medication information with the medical team, nursing staff and pharmacy staff. There is opportunity for an 8 on-6 off schedule (10 hour shifts) or for a regular 5-day schedule (8 hour shifts).
